The need for home care services often highlights a time of life when people are in decline. They are losing control of the life they knew and family members often are at a loss to help them. Baby Boomers find themselves sandwiched between caring for their own children and their aging parents. It can be difficult to do both, especially when elder loved ones are miles away.
When our caregivers are hired, we include our Care Management services at no additional fee to supervise our caregivers and offer solutions to problems should they arise. We know that we are often the “eyes and ears” for family members and our ongoing management of our caregivers gives our clients “peace of mind”. Often, we are able to take on the problem, offer solutions and sometimes solve issues prior to contacting you. We work with families through phone calls, visits and emails.
Our goal with providing home care in San Francisco is to keep a person at home as long as possible and then help their move when they can no longer be safe at home. We are able to check on problems that come up with providers and work with all parties to provide the best possible care. Home visits make us proactive instead of reactive.. We can refer clients and families to Independent Geriatric Care Managers who are available to provide more extensive services.
Aunt Ann’s Homecare does not charge a fee for our Care Management Services.
Our Process – The Personal Care Plan
Our Nurse Supervisor will visit with you in the hospital or at home and perform an assessment of your needs. We don’t charge a fee for this initial assessment. We will also meet with families to strategize the care needed for a family member, prepare an assessment, and make recommendations. When Aunt Ann’s Home Care is hired, we will draft a written assessment and a personal care plan for our caregivers to follow. This personal care plan serves as a guide to implement and supervise the caregivers and the care provided on an ongoing basis. Caregivers will complete weekly work sheets, and adjustments to the care plan will be made when necessary.
