Services: Care Management Services The Difference Aunt Ann's Elder Home Care recognizes Care Management services ensure superior care. We include these services at no additional cost. Our Care Management System provides the necessary checks and balances. This gives our clients "peace of mind", knowing their loved ones are receiving the best care. Our Process - The Personal Care Plan A Nurse Supervisor will visit with you in the hospital or at home and perform an assessment of your needs. Once we understand your needs, we will initiate a personal care plan for our trained elder care aide to follow.
