Services: How To Getting Started
Are you considering Eldercare or Home Care Assistance? Have you been told by the hospital that your loved one or yourself can not return home without household help? Unsure about how many days or hours you may need? Do you want to discuss how a eldercare or home care caregiver can assist you in your recovery or provide long term assistance to remain in your home?
Aunt Ann’s Home Care can assist you to remain independent in your own home. Who better than a trusted agency that has been providing eldercare and home care caregiver services in San Francisco Bay Area homes since 1958.
First Step: Free In-Home Assessment
Contact Aunt Ann’s Home Care for a free in-home or in-hospital assessment by our Care Management Staff. During this meeting our Care Manager will assess your individual needs and make recommendations that match your needs.
Second Step: Develop an individualized Care Plan
Aunt Ann’s Home Care Care Manager prepares and reviews a comprehensive care plan of care which includes:
- Household care: cooking, cleaning, laundry, shopping
- Personal care: bathing, eating, dressing, toileting,
- Health care: co-ordination of services with physicians, pharmacy , home health care
- Emotional care: companionship, conversation, activities
Third Step: Select Caregiver(s) and Begin Orientation
Once you and our Care Manager have agreed on a care plan , then it will be time to select your caregiver(s).
Our scheduling staff will come up with the most appropriate caregiver and schedule a interview with you.
After the meeting with the caregiver, if you feel comfortable services will begin. You may change your caregiver at any time by contacting your Staffing Manager.
Aunt Ann’s Home Care will orient the caregiver(s) and your assigned Care Manager will make home visits to supervise your caregiver and make sure that your eldercare or homecare caregiver is following your care plan and that you are satisfied with the services.
